I misunderstood here.  I thought the bug affected upgrades from 8.3 to 
8.4 using the normal Debian pg_upgradecluster method (and so was holding 
off on my lenny to squeeze upgrades).  Now I see that it only affects 
upgrades specifically done with pg_upgrade.

TOAST tables are transparently used when you store a large amount of 
data in a field (aything over 8k per row).  They aren't specifically 
configured or require any special syntax, so I'm guessing many folks are 
not aware their databases contain them.
