I noticed (eventually) that I was getting no
outside email. Here are the last things logged:

Feb 23 01:29:43 wren postgrey[27021]: cleaning main database finished. before: 
74074, after: 57321 
Feb 23 01:29:43 wren postgrey[27021]: FATAL: Can't call method "txn_commit" on 
an undefined value at /usr/sbin/postgrey line 306.  

postgrey was no longer running.

Perhaps fixing or at least avoiding this crash is as simple as checking that
$db_env->txn_begin returns a $txn object? OTOH, I don't understand why
the 2 lines above were logged in the reverse order that would be expected.
