The installer and debootstrap will install essential packages.

The installer is not responsible for upgrades.  Transitional packages
are necessary to ensure that new essential packages are installed.
In this case, /bin/mktemp used to be in the debianutils package until
the Sarge release but was moved to the new mktemp package in Etch.  The
debianutils package in Etch pre-depended on mktemp, but this dependency
was removed after the Etch release.

Most likely this happened because you upgraded debianutils from Sarge to
a post-Etch version.  Such upgrades leaping over a stable Debian release
are not supported.
