Sid should have VTK 5.4 now.

Anyway, yes Caret is broken. This is documented here:

http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=522486
http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=520701
http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=525789
http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=554061

All these bugreports are reflections of the same problem. Caret is
outdated, and currently upstream refuses to update the code to be
compatible with recent library versions (e.g. VTK).

I tried starting a patch, but soon realized that it goes deep into the
core, and I wouldn't have a clue what I would break.

I'm hoping that at some point upstream agrees that it is time to move on
-- let's hope.
