when I ran it inside screen I didn't get the prompt at all.
I exited screen, logged out of the server, logged back in, re-ran
dist-upgrade and this time I did get the prompt.
something is fishy with this prompt.

You should be getting a debconf prompt _somewhere_. The original
submitter's problem was that he didn't see it.
