I just installed phpldapadmin 1.1.0.5-1, and get the same problem. I looked in my vhost logs and saw no errors, but on a hunch I looked in the main logs, and found this being logged for every request:
[notice] child pid <pid> exit signal Segmentation fault (11) ...which would probably explain the badness. Doesn't tell me which process though...
