I'm sorry, but I don't see this problem at all on my systems, and I did test
fairly extensively before uploading.

To help you fix this, I will need to know the contents of your
/etc/pam.d/login and /etc/pam.d/common-* files.

Are you completely locked out of the machine?  If so, you will probably need
to reboot with the 'single' option to get a single-user shell on the system,
or you might even be better off booting from rescue media depending on how
badly broken things are.  Once you're able to log in, I would suggest that
the first thing you should do is downgrade the libpam-modules and libpam0g
packages, so that you can get back to using your system while we continue
debugging this problem.

For information, all of the following modules have been tested, at least in
their default configurations, prior to my upload:

  pam_securetty, pam_nologin, pam_env, pam_group, pam_limits, pam_lastlog,
  pam_motd, pam_mail, pam_krb5, pam_unix, pam_selinux, pam_rootok,
  pam_access

so your configuration seems to be quite extraordinary after all if it
depends on a module not in this list for login.  Again, I'm sorry that it's
broken for you, but I will definitely need more information to figure out
why.
