This problem has been fixed with the release of (lib)parted 2.4 and higher. The relevant git commit that addressed this "Assertion (head size <= 63)" problem can be found at the following link:

Remove PED_ASSERT from dos geometry checking
http://git.savannah.gnu.org/cgit/parted.git/commit/?id=244b1b25a12198efb076e8c65be77b5750776583

This problem was also reported in Ubuntu and tracked under the following bug report:
GParted crashes with Assertion (head_size <= 63)
https://bugs.launchpad.net/ubuntu/+source/gparted/+bug/545911

The work-around to this problem is as follows:
1) Copy your data to another device
2) Write a new partition table to the disk device
3) Restore your data back to the usb device.

Regards,
Curtis Gedak
