If I understand this correctly, the auto cleanup only kicks in after an 
installation, so that if you want to clean-up the work directory after some 
weeks of inactivity (e.g. I've installed some modules three weeks ago, and
never touched cpanm after that) you have to install (or upgrade) a module, 
which is not possible in case of missing internet connectivity. Is this
correct?
