Could you try switching to VT2 near the beginning of the installation and adding a line "set -x" near the top of the file /var/lib/dpkg/info/rescue-mode.postinst That should give you debugging output in /var/log/syslog. You can get that information off the system using the "Save debug logs" option in the main menu. Also, are you sure that you've selected the correct device to mount as root filesystem?
