I could not reproduce this problem at least not with an almost empty
DITA topic file. Does it only occur in certain conditions, such as large
files or otherwise big load?

Note that the open source Serna edition seems to be relatively abandoned
by Syntext (mailing list dead etc.), and this has a negative effect on
my motivation as the maintainer. This means that some issues will only
be fixed if you (or someone else) provides a fix :-)
