There is no problem in my machine. I tried to play your file with
madplay, mplayer and others and it has a high-level background noise and
high deformation with every player, as with mpg321. On the other hand, nothing 
has
changed in mpg321 that has to do with the output stream or the mad
library that manipulates the mp3 stream. Even the changes to the ao
library has nothing to do with the audio output.

Maybe the problem is the encoder or the application you use to create
the wav file at first or the mp3 file later.

There are no changes regarding the audio stream or audio output between
0.2.11-3 and 0.2.12-1.

Do you have the same problem with every mp3 file?
Unfortunately I can't reproduce your problem and the attached mp3 file
seems to have the same problem as I said with every other player used until now 
in
my machine (amd64).
