I have been setting up debian testing on an older 19 inch monitor with the 3dfx driver (tdfx). If I used the simplified monitor setup, and report that I have a 19-20 inch monitor, but only use display modes to 1280x1024, the XF86Config-4 file will allow Hsync from 30-160 and Vsync from 50-150 (necessary for the 2000x1500 resolution, I guess). As a result, the automatic mode selection will choose Hsync and Vsync which exceed my monitor's capabilities in 1280x1024 mode. This happens in both of the simplified monitor setup modes (Simple and Medium). In advanced mode I could force Vsync and Hsync to reasonable values for my monitor, and everything came up. I think I was led astray by the simplified setup -- In particular, the max vsync rate > 85 is surprizing to me.
