There is a problem in yout thinking...

"realname" must be set BEFORE the hook becomes active.
This is NOT ab bug but a feature.

Also the use of <SPACE> arrond the "=" lead sometimes to errors:

> reply-hook "~t 1...@home.tld" 'set realname = "set-realname-1"'
> reply-hook "~t 2...@home.tld" 'set realname = "set-realname-2"'
                                          ^^^
Maybe you sould use:

reply-hook "~t 1...@home.tld" 'my_hdr From: set-realname-1 <1...@home.tld>'
reply-hook "~t 2...@home.tld" 'my-hdr From: set-realname-2 <2...@home.tld>'

which is DEFINITIVELY working since "my_hdr" exist and I use it daily.
