I had a very similar problem after upgrading initramfs-tools from
0.93.4 to 0.94.4.

My system uses LVM2 on top of an encrypted block device. Therefore, I
have to enter a passphrase as soon as the filesystems are to be mounted
during the boot process. But my keyboard wasn't working anymore because
hid_microsoft.ko was missing in the newly generated initrd although I am
using "MODULES=most".

I fixed this temporarily by adding hid-microsoft  to
/etc/initramfs-tools/modules, updating the initrd afterwards and
rebooting.

Please rethink your change "Revert "hook-functions: Add hid_* modules.""
you made in 0.94. I prefer a bloated initrd more than a non-booting
system. Especially when I'm in a hurry. ;-)
