Hi,
I cannot reproduce this with mutt 1.5.20-6, this is what I did:

shell$ cat .muttrc-realname
set realname=bar
reply-hook . set realname=foo

shell$ mutt -F .muttrc-realname

then I reply to a message and the realname is correctly set to "foo".

Please give us more info about how to reproduce this bug or try this
sequence yourself and see if it's still reproducible.
