iscsistart within an initramfs suffers from an unreliable parent<->slave
communication setup sequence -- this is fixed in upstream git commit
2f1a3c897090e6fadc36288e01891dfe82a7bb89 dated 2009-04-12.

This bug reliably affects kernels >= 2.6.32 -- in 2.6.31-trunk
all is well -- the race is won 'properly' ;-)
