Maybe something went wrong during a given upgrade, but it's hard to tell. Or maybe your permissions.sqlite file ended up in bad shape for $RANDOM reason. If you haven't manually fixed you configuration yet, or have a backup of your broken configuration, maybe we could take a look at that file.
