I just get this from time to time with daily images. Another user reported the same issue on IRC yesterday. This is really annoying and definitely a regression from lenny. It happens with SD cards, USB stick and SATA disks... it doesn't happen all the time, but quite frequently. I didn't see anything wrong in /var/log/syslog or dmesg.
