If you use -M exec /usr/share/smartmontools/smartd-runner this is expected. It is a specific Debian script and it launches every script in /etc/smartmontools/run.d/ instead of the hardcoded /usr/bin/mail. Try to remove the -M exec option, and you will get a warning in your syslog. Cheers, Giuseppe.
