Could you upgrade to 1.6.8 and see if that is better. Checksum database handling was changed in 1.6.5. At this point you need to run /usr/share/apt-cacher/apt-cacher-cleanup.pl -r after killing the processes but before restarting to clean locks and mutexes from the checksum database.
