You also need the hd-media kernel and initrd for amd64 (which seems 
obvious to me).
After that all that is really needed is correct configuration of syslinux 
(or whatever bootloader you wish to use). You should be able to do that 
by studying the syslinux configuration of the multi-boot CD.

IMO this is rather outside the scope of the manual. You are welcome to 
document it on the wiki if you figure it out.

There are vague plans to change the way USB sticks are created (using a 
script that sets things up on demand). Possibly m-a could be supported in 
that.

This is already documented in the section on boot parameters.
