info/dir very easily loses consistency with what is on disk.

The install-info man page should have a section to answer:

Q: How do I refresh the entire /usr/share/info/dir to reflect what is on
   my system with just one command?

A: SEE ALSO rebuild-info(8) (please implement)

You see, there are stale entries there like
* BHL: (bhl.info).    BHL mode: Plain text - HTML - LaTeX -
etc. for packages no longer installed, whose entries for some reason did
not get cleaned up.

Rather than relying on each package to clean up after itself, perhaps
just rebuild the whole thing each time anything changes?
