The fallback to shell authentication / authorization is beyond the
scope of pam-mysql. If there is a line in eg.

 /etc/pam.d/ssh, which says:

 auth sufficient pam_mysql.so

And pam-mysql authenticates the user, then pam-mysql works. You need
to correctly add a fallback rule in pam.

Feel free to paste your problematic pam.d/FOO-file here along with
yout fallback wish - stated in plain english, so I may give a a
suggestion.
