I'd like an option for ssh-copy-id that would login to a remote host and
replace key X by key Y.

ssh-copy-id --replace ~/new-ssh-key.pub -i ~/old-ssh-key.pub vps.somehost.org

It should be robust enough to not cut off access to the server in any
case at all. The old key must not be removed from the authorized_keys
file until it has been verified that the new key works as desired. The
commands used must not break the structure of the authorized_keys file
and should preserve all whitespace, comments etc.
