I think *-patch-missing-description should mention more things and I
propose the following replacement text. The things I mention are very
useful for reducing divergence in cases where the Debian maintainer
hasn't done a very good job of forwarding patches upstream or upstream
hasn't been active or accepted the patches.

I: foo source: quilt-patch-missing-description foo.patch

N:    quilt patch files should carry a description of the function of the
N:    included patch. Description lines may be included before the start of
N:    the patch itself. Other recommended metadata include authors, a URL for
N:    where the bug was taken from, Debian/upstream bugs closed, upstream 
status,
N:    Debian version & date the patch was introduced and URLs to initial reports
N:    or other interesting info.
