I have a reproduceable (100% of the times I've tried) rxvt command line
instance that will cause rxvt to not exit when the captured (child)
process exits, in this case, mutt:

rxvt -g 80x46+1620+20 -rv -cr yellow -fn fixed +sb -name mail -e mutt

I suspect that the exact geometry numbers aren't important, if you
don't have a screen that big.  When you quit out of mutt, rxvt stays,
can be iconized and windowshaded, but xkill, while it gets rid of the
window, does not kill off rxvt.  I'm running wmaker.  This under sarge
with a custom built 2.6.11.11 kernel on a dual Athlon system.  It does
not exhibit this behavior on 2.4 kernels, at least not often enough
to remember.  I have just switched to using 2.6 kernels on this machine
in the last few days, but I've been using this desktop for years now.

The LD_ASSUME_KERNEL=2.4.1 workaround does work.
