I've had the same problem, which went away immediately after enabling kernel
mode (pppoe-server -k). To do that you first need to get #298185 fixed
though. I've just sent a proposed solution to that bug.

I have no explanation on why the non-kernelmode pppoe-server responses are
getting lost, but I saw the same pattern: pppd thinks that it's sending
replies, but the interface on which they should be going out is not sending
anything. I may do some further research on that later but first I've got one
more pppoe bugreport to compose. It'll be coming in a few minutes.
