/usr/lib/mon/alert.d/ipvs.alert /usr/lib/mon/mon.d/ipvs.monitor respectively. I will submit them to upstream, as well. They are a minimal overhaul from Christopher DeMarco's work back in 2004 and can be used to monitor & change the state of ipvs (see Debian package ipvsadm for reference).
