After adding a Promise disk array, that had just finished a RAID init, I
was able to partition and create a file system. However, after a few
writes the device failed, from the linux point of view. An attempt to
issue a echo "scsi remove-single-device 2 0 0 0" > /dev/scsi/scsi
produces the kernel trace. The system has to be rebooted as others have
reported.
The system is a stock Dell Power Edge 2850
