Personally, I think for most gnome-orca bugs it is probably best to report them upstream at bugzilla.gnome.org. The orca team is pretty responsive. Besides, most accessibility bugs can be fixed in gnome-orca directly (by scriptability) and if the bug really belongs to an application, the orca team usually is good at forwarding stuff to the appropriate people. It also makes sense to check with other users first if someone else observes the same behavior. I personally dont use OOo yet since my first attempts of using it were very disappointing since many things were not working.
