Simply spelling of text with Festival doesn't work. As a partial work around you can make your application / screen reader to use the Speech Dispatcher character reading function (on each of the characters of the given piece of text) instead of the spelling function. But this way the punctuation settings don't apply. I'll ask upstream about status of the bug. Thanks for the report.
