I've just tried MayaVi GL2PS image saving on a Debian testing/unstable
system with an Intel 965G integrated graphics chipset.

I get the same exact behavior as described above (including the
remaining issues) with the following package versions:

mayavi                               1.5-5
xorg                                 1:7.2-5
linux-image-2.6.24-1-amd64           2.6.24-5
xserver-xorg-video-intel             2:2.2.1-1
