I have to admit, I've never seen this before, but I don't have access to a LVM machine anyway. However, the linux-boot-prober output looks sane to me; it's separated by colons and all space characters have been substituted by circumflexes. I believe that the first two sections of the output are wrong, i.e. boot and root device; but that's what Philip allready suspected.
