Despite the fact the fact that this could cause pppd to hang, I have
only given this bug a severity of normal, since it should only happen on
improperly configured systems.

The problem is that if mfd is opened, but its associated sfd cannot be
opened for any reason, then the mfd will not be closed, and an attempt
will be made to re-use mfd to open another PTY.

I found this problem while trying to search for a PTY leak while using
the pty command, but this is not the problem. If no patch is produced in
the next week or so, I will try to submit a patch to resolve this
problem.
