For point 1, there is a known bug that evince does not actually print if
it is closed before the printing job is created. Is this bug which
affects you?
I don't think it's exactly this bug. In my case, even Print Preview
doesn't show up, no matter how long I wait.

Happy to say that the 2nd point is fixed too in trunk (see
http://bugzilla.gnome.org/show_bug.cgi?id=365332).
For the 1st point, can you find a reproducible method to trigger the
bug? I have printed and I haven't seen your bug. Also, are you willing
to compile yourself evince and add a few printf to see where the bug
comes from?
(I do not know if I must put the "forward" control BTS command, sorry.)
