Package: phpmyadmin
Version: 4:2.6.2-1
Severity: important

While updating I had the following:

Replacing config file /etc/apache2/conf.d/phpmyadmin.conf with new version
Replacing config file /etc/phpmyadmin/htaccess with new version
Forcing reload of web server: Apache2 ... failed!
You may still have some apache2 processes running. There are
processes named 'apache2' which do not match your pid file,
and in the name of safety, we've left them alone. Please review
the situation by hand.
apache2: could not open document config file /etc/phpmyadmin/apache.conf
invoke-rc.d: initscript apache2, action "restart" failed.
dpkg: error processing phpmyadmin (--configure):
subprocess post-installation script returned error exit status 1
Setting up webmin-firewall (1.180-4) ...

resulted in renaming of apache.conf in apache.conf.dpkg-old but NOT
installing a new one. Resulted in shutdown of apache. Solution was just to
copy old version to apache.conf
