Isn't that also a bug in insserv?
Imho update-rc.d from insserv should always keep the information
provided by the LSB header and the symlinks in sync.
I.e. if the LSB header says
Default-Start: 2 3 4 5
Default-Stop: 1
the symlinks in 0 6 should be removed automatically by insserv/update-rc.d
