This patch would be very nice to include. One thing to pay attention to is that the "-n standby" parameter for smartctl is only available since smartmontools 5.37 (etch has 5.36). So I think the debian package could include this patch and add versioned Conflicts and Suggests for smartmontools.
Regarding upstream, there is a related task: http://munin.projects.linpro.no/ticket/137 I had reopened it and submitted a patch before I saw this debian bug report. My patch uses hdparm -C, which is less elegant, but it will work with older versions of smartmontools, so maybe it is more appropriate for upstream at the moment.
