Unfortunately, I missed this one when I've adapted the package for
debian. Gnome-phone-manager depends on gnome-icon-theme >= 2.19, this
one is not yet in debian unstable so I've managed to include the icons
needed by gnome-phone-manager in the deb. In the next upload I try to
fix this.

Now I think this issue solved... Can you try to update to
gnome-icon-theme 2.20.0? How it appears?

I assume this issue is solved, I've checked on my box after the upgrade and it display
the right icon.

2.) The button below 'Bluetooth' on the connection tab does not have any
    label when no phone is selected, which is very confusing.

I think this one it's not related to the packaging, I check it ASAP and,
in case, I'll contact the upstream or I'll try to fix it.

For this I'm trying to add a label like "choose bluetooth device", I
hope to fix this one for the next release...

I'm working on it, but in this period I was away many times, I forward to upstream author,
however sight the nature of the bug I lower the severity to whislit.
