For make-kpkg, I used the following (with pretty much the stock
kernel config from linux-image-2.6.22-2-amd64_2.6.22-4_i386.deb):
make-kpkg -revision=1.tp --append-to-version=-amd64 --arch amd64 --
cross-compile - --initrd kernel-image
and
make-kpkg -revision=1.tp --append-to-version=-amd64 --arch amd64 --
cross-compile - --initrd modules
