Please note that a newer version of this file (t/lib/if.pm) is already included in the Perl core distribution as /usr/share/perl/5.8.8/if.pm, and the author is a well-known Perl guru/hacker. Presumably the lack of license is thus just an oversight. Bug reports might be in order. The module is on CPAN too, FWIW.
