Dear Ralph, Norbert, I prepared a sample of the text for you guys today, compiled it, it worked. (!) Went back to the main bookk text, compiled it again, it worked. (!!) I am pleased to say (since it involves no work on your parts) that I seem to have been making some sort of systematic error in the compilation process which I have not been able to recreate. (Have just spent half an hour trying to get it to do it again - no joy.) If I find out that there is something, I will be in touch again, but please close the bug and accept my apologies and thanks for your trouble.
