I run amavis under user/group amavis, and clamd under user/group clamav. The clamav user is also in the amavis group. When a mail comes in, amavisd-new-milter creates a directory in /var/lib/amavis, with permissions 700 (owner amavis). This makes it inaccessible for clamav. I'd recommend setting it to 750.
