Running webcam(1) in continuous-upload mode eventually causes the ftp(1)
subprocess used to run out of file descriptors, returning 'fdopen()
failed' to the webcam process.

webcam(1) continues to attempt to use the same ftp(1) process when an
error occurs, but as this process is out of file descriptors it will
never recover. Instead, webcam(1) should probably attempt to spawn a new
ftp(1) process.

webcam(1) also fails if the ftp(1) subprocess is killed - while
understandable, the ability to restart ftp(1) could be added to this
code path at the same time.
