There are two entirely seperable issues here.  First, make sure that
what you have in /var/lib/clamav actually needs to be there by running 
rm -rf /var/lib/clamav/*
freshclam -v

Then, a seperate issue: WTF are you invoking clamscan seperately for
each email?  Use clamdscan and consider getting just a little
performance out of the software you use.
