There does seem to be a config option that affects this because my setup works as one would expect. The text is inserted on all selected lines. I'm not quite sure what's different about my setup that makes this work as I haven't had time to check my various settings. Testing vim7 without any config and in compatible mode (vim -u NONE -N) exhibits the behavior you describe with CTRL-C where text is only inserted on the first line of the visual block.
