The generated initramfs image doesn't contain the /sbin/mpath_prio_* helper programs, thus multipath bails out when invoked from the initramfs at boot time. Adding copy_exec /sbin/mpath_prio_emc /sbin/ to /usr/share/initramfs-tools/hooks/multipath_hook fixes the issue for me, as a general solution, copy_exec /sbin/mpath_prio_* /sbin/ would probably be better.
