The Debian (Xen) kernel images are compiled with CONFIG_LEGACY_PTY_COUNT=16. This number is extremely low, and you hit that limit very fast, which results in various problems. >From the Linux kernel configuration: The default is 256, and should be more >than enough. Is there any particular reason for setting this to 16 instead of leaving the default value? Please consider increasing this value in the Debian kernel images to at least 64, preferably the *default* value.
