The icon cache use has been discussed several times with the Debian GNOME Team before any work started on that for Ubuntu and there is no mistake which has been done or to "avoid". That was clear from those discussions than pkg-gnome was not wanting to use the current cache implementation and that people are busy enough to not work on that at the moment. The Ubuntu Desktop Team on the other side wanted to get the icon cache used, has no problem with the current implementation and was fine rebuilding the packages installing an icon to update the cache.
